Application
You can enroll in the Charter Oak Health Plan by downloading the Charter Oak Health Plan Application Form, completing it and mailing it to the address on the form.
Note: The file is available in Adobe Acrobat format to download. The Acrobat PDF format makes the application very easy to print and and fill out off-line. If you don't have the Adobe Acrobat reader, you can download it here for FREE.
You can also download the file in Word format, complete it, and mail it to the address on the form.
If you have problems downloading the application form, or you do not have Adobe Acrobat Reader you can call 1-877-77-CT-OAK to request an application be mailed to you. Customer service representatives will also be happy to help you with the application process.
